| MADURA MAYHEM T-SHIRTS AND BACKREST BRACKET ORDERING INFORMATION

Madura owners:
The flyer which I put together below shows the shirts that are available and pricing, as well as information about the V1200 backrest bracket. In order to see this flyer, you must right-click over this .pdf flyer link, then choose the "Save Target As..." or the "Save Link As..." option from the pop-up menu to download it to your hard drive. Remember where you save it! Then browse to that location and double click the file to open it in Adobe Acrobat Reader, which is a free download from adobe.com if you don't have it!
To fill your order I need your help first. This makes it easy for me to fill your order in the fastest and organized way. It is also very important that if you want a special size to get it to me as soon as possible. I need to know what shirts you want by name (Madura Road/Polisher’s, 2006 shirt or 2005 shirt) I need to know what colors if you want something different.

B) Quantity
C) Color (in no color specified or after May 9 it will be black)

Once you email me the above information I will send you the total of your invoice that will include postage. You can then send in your order. Sorry but at this time only money orders and personal checks are accepted. If you would rather pick up your shirts at Mayhem that will also be possible.
